They trick Aldo, the guard and force him to tell them where Karl is and release him. Trying to fix up Ben, Jack knicks an artery. When Pickett is about to shoot Sawyer, Juliet kills him. While Kate tells him the count to five story, Jack completes the surgery on Ben and tells Kate not to come back for him. Kate, Sawyer and Karl leave Hydra Island. Juliet tells Jack what Ben said.
- Kate begs Sawyer to turn around to get Jack, but Sawyer refuses. Tom moves Jack to the cages while Juliet is put in the aquarium. Kate and Sawyer make camp on the Island but continue to argue. Karl tells Sawyer and Kate that the people they kidnapped are being given a better life than the other survivors, and reminisces about his developed home.
- That evening, Charlie and Hurley try to find out how Desmond can see the future. They offer him MacCutcheon whisky and after he is drunk, ask him how he knew Claire was drowning and about the lightning. Desmond pins Charlie telling him he doesn't want to know what happened to him, but eventually tells that he has been foreseeing Charlie's death, but cannot prevent it indefinitely.
